 Pediatric Advanced Life Support (PALS) Provider Course Documentation Policy for Shell CPR, LLC 

To uphold the standards set by the American Heart Association (AHA) and Shell CPR, LLC, all required documentation for Pediatric Advanced Life Support (PALS) Provider courses must be completed accurately and uploaded promptly into the Enrollware system after each course.

All instructors are responsible for ensuring that full and accurate course documentation is submitted to the Enrollware system within 48 hours of course completion. This documentation is critical for student certification, quality assurance, and compliance audits.

1. Completed Course Roster
    
   • Must include printed names, email addresses, and phone numbers of all participants
      
   • Signed and dated by the lead instructor
      

1. Skills Performance Evaluation Sheets
    
   • Required for each student, evaluating the following skill areas:
      
     • Pediatric High-Quality CPR
        
     • Airway Management
        
     • PALS Megacode Simulation
        
     • Team Dynamics
        
   • Must be signed by the evaluating instructor or faculty
      

1. Written Exam Answer Sheets or Score Summary
    
   • All participants must complete and pass the written exam
      
   • Submit either the original answer sheets or a documented score summary
      

1. eLearning Completion Certificate (for HeartCode PALS / Blended Learning Courses)
    
   • Required for students who complete the online portion prior to the hands-on session
      
   • Each certificate must be uploaded and correspond to the course date
      

1. Instructor Monitoring Form (if applicable)
    
   • Required when monitoring new or renewing PALS instructors teaching during the course
      

1. Incident Reports or Special Notes (if applicable)
    
   • Any incidents, disputes, or course-related concerns must be documented and attached
      

1. Scan all required documents into a legible format (preferably a single PDF file)
    
2. Upload the file(s) to the Enrollware system under the correct course listing and date
    
3. Clearly label each upload (e.g., “PALS_MM-DD-YYYY_InstructorName”)
    
4. Retain original physical copies for a minimum of 3 years as required by AHA guidelines
    

Timely and accurate documentation is required to maintain instructor alignment with Shell CPR, LLC. Non-compliance may result in:

• Temporary suspension from teaching
   
• Delay in eCard issuance
   
• Permanent revocation of instructor status for repeated violations
