Advanced Cardiac Life Support (ACLS) Provider Course Documentation Policy for Shell CPR, LLC
Purpose:
To ensure full compliance with American Heart Association (AHA) requirements and Shell CPR, LLC’s documentation standards, all ACLS Provider course paperwork must be completed, scanned, and uploaded into the Enrollware system following each training session.
Policy Statement:
Instructors are required to upload all necessary documentation for each ACLS Provider course to the Enrollware system within 48 hours of class completion. This documentation is essential for verification of training, issuance of certification, and record retention purposes.
Required Documentation for Each ACLS Provider Course:
- Completed Course Roster
- Must include printed name, email address, and phone number for each participant
- Instructor signature and accurate course date required
- Skills Performance Evaluation Sheets
- Each participant must have completed and signed evaluation forms for required ACLS skills, including:
- High-Quality CPR
- Airway Management
- Megacode Simulation
- Team Dynamics
- Written Exam Answer Sheets or Score Summary
- All students must complete and pass the written exam
- Submit answer sheets or exam summary forms with scores clearly documented
- eLearning Completion Certificate (for HeartCode ACLS / Blended Learning)
- Required for students completing the online portion of the ACLS course
- Certificate must show full completion prior to hands-on session
- Instructor Monitoring Form (if applicable)
- To be completed if the course includes monitoring of a new or renewing ACLS instructor
- Special Documentation or Incident Reports (if applicable)
- Include documentation of any disputes, incidents, or special accommodations
Submission Process:
- Combine all scanned documents into a single PDF or neatly organized folder
- Upload the documentation to the Enrollware system under the correct course listing
- Label uploaded files with clear identifiers (e.g., “ACLS_Course_MM-DD-YYYY_InstructorName”)
- Maintain original paper documents in a secure location for a minimum of 3 years as per AHA record retention policy
Compliance & Disciplinary Action:
Failure to submit accurate and timely documentation may lead to a formal review. Continued non-compliance can result in:
- Temporary suspension from teaching
- Revocation of instructor alignment
- Denial of eCard issuance for incomplete files
