Purpose:
To maintain compliance with American Heart Association (AHA) guidelines and Shell CPR, LLC documentation standards, all required paperwork for each Heartsaver First Aid CPR AED course must be accurately completed and submitted via the Enrollware system promptly after course completion.
Policy Statement:
Instructors must scan and upload all required documentation for each Heartsaver First Aid CPR AED course to the Enrollware system within 48 hours of the class. Timely submission ensures accurate recordkeeping and upholds the integrity of Shell CPR, LLC’s training program.
Required Documentation for Each Heartsaver First Aid CPR AED Course:
- Completed Course Roster
- Includes full name, email address, and phone number of each participant
- Instructor signature and course completion date required
- Skills Performance Checklists
- Signed skills checklists are required for each student who completes the course
- Must reflect all required skill components (CPR, AED, First Aid as applicable)
- Written Exam Answer Sheets or Score Summary (if required)
- If exams are given (as per organizational policy or at the instructor's discretion), answer sheets or score summaries must be submitted
- eLearning Completion Certificate (for Blended Learning Courses)
- Participants who complete the online portion must present a certificate of completion
- This must be uploaded for each student participating in a blended format
- Instructor Monitoring Form (if applicable)
- Required when a new or renewing instructor is being evaluated during the course
- Incident Reports or Special Documentation (if applicable)
- Any concerns, special accommodations, or unusual occurrences must be documented and attached to the course file
Submission Process:
- Scan all course documents into a clearly organized file or folder (preferably PDF format)
- Upload documentation to the Enrollware system under the appropriate course listing and date
- Label files clearly (e.g., “Heartsaver_FirstAidCPRAED_MM-DD-YYYY_InstructorName”)
- Retain original hard copies in accordance with the Shell CPR, LLC document retention policy (minimum of 3 years)
Compliance and Enforcement:
Failure to submit accurate and timely documentation may result in suspension or revocation of teaching privileges. Repeated violations of this policy will not be tolerated and may affect instructor alignment status with Shell CPR, LLC.
